I was able to reproduce it at will, but it looks like I misread the cause. When I had all three 220's chained together on the same S-link bus (as player 1, 2, and 3), they were apparently interfering with each other occasionally. The most obvious symptom was that trying to have CDJ turn them all off on exit left player 2 on. They were definitely configured as different players, and they operated correctly in a playlist, but obviously they were picking up at least an occasional signal directed at another player.

Splitting them out onto separate s-link busses not only cured that problem, but also the track-skipping. My guess is that the command to play the second track from the same disc was happening very close to the command to pre-queue the disc for the following track, and the active player picked up enough of it to return something that CDJ interpreted as a reason to move on.
